1 Proceeds to Customer Service Division Interviews applicant and gives list of requirements 10 Minutes Customer Service Officer B / Customer Service Assistant A Checklist Requirement
After completion of Requirement indicated in the Checklist
2 Submits requirement to Service Application Personnel

Check requirements. If complete, furnishes applicant / client with Service Application Form / Change of Name. Requires applicant to attend Orientation Seminar

 Customer Service Officer B / Customer Service Assistant Service Application / Change of Name Form
3 Proceeds to attend Orientation Seminar at ZCWD roof deck Conducts Orientation Seminar 2 Hours Orientation Seminar Committee Entry pass

After the seminar, proceeds to Cashier for payment of inspection fee

 Accepts payment and issues official receipt 2 Minutes Cashier Certificate of Seminar
5 Proceeds to Service Application Section and submits the completed form together with the O.R. Evaluates documents. If complete, submits a copy to the applicant and processes request. 20 Minutes Customer Service Officer B / Customer Service Assistant Change of Name Form
